This form contains two (2) important documents that need to be carefully reviewed and acknowledged before a student will be allowed to participate in Mountain View School Athletics.
As a parent of a student/athlete representing Mountain View School, it is expected that you role model behaviour conducive to good sportsmanship and behaviour that will not embarrass the school, your student, or yourself. This form must be filled out by parents before a child is permitted to participate in Mountain View Athletics.
